Release: We (I) hereby submit this application for enrollment to the Baseball Academy of Tulsa (BAT). In your acceptance, we (I) hereby release the BAT staff, anyone appointed by BAT to help, from all claims of any injuries sustained by our (my) child during camps. We (I) also agree to indemnify said staff and other employees for any claim hereafter as a result of such injuries. As parents/guardians, we (I) also acknowledge there is an assumed possibility or risk whenever a student participates in baseball. (If possible, both parents should sign form). |

| *All campers must pre-registered by Sunday March 14th. To register by phone call 724-9228 or 724-9966. Each application must be completed and signed by parent or guardian. No refunds issued within 7 days of camp. Any camper who leaves camp due to illness/injury will receive a prorated credit good for future camps. |
